Individual will work under the direction of the RN and/or Social Worker to complete referrals for post acute services and assists with tasks that do not require a clinical license or a degree. Responsibilities include:
Arranging Post-discharge services
Creating and following up on electronic referrals using the Tenet Case Management System.
Reviewing Patient choice letters with patients/families for required signatures.
Providing follow up improtant message to Medicare patients prior to discharge.
Communicating with patients, families and other members of the care team.
Completing tasks addigned by Case Manager and Social Work staff.
Completing Process Reviews or Audits as requested
High School Graduate - Bachelor''s degree is preferred. Paramedic, EMT or CNA license is also preferred. Also, should have acute hospital experience to perform competently.
The position requires excellent organizational skills, demontrated problem solving abilities, computer literacy as well as excellent verbal and written communication skills.Estimated Salary: $20 to $28 per hour based on qualifications.
