JOB SUMMARY

Provides accurate and timely assignment of appropriate diagnostic and procedural codes on the medical records for the purpose of collecting and indexing quality health information for routine patient types (outpatient diagnostic, outpatient physician practice/clinic, and/or emergency room encounters).

EDUCATION

Associate Degree in Health Information Technology

CERTIFICATION SUMMARY

One of the following:
Certified Coding Associate (CCA), Certified Coding Specialist- Physician-based (CCS-P), Certified Professional Coder (CPC) or Certified Professional Coder-Hospital Outpatient (CPC-H)

EXPERIENCE

  • 1 year of experience in hospital and/or physician practice outpatient coding

PREFERRED QUALIFICATIONS

  • Associate's degree in Health Information Management or related field

KNOWLEDGE SKILLS & ABILITIES

Demonstrated knowledge of medical terminology, anatomy and physiology, pharmacology, coding guidelines, and computers
Proven detail orientation and good problem-solving related to coding

JOB RESPONSIBILITIES

  1. Reviews the medical record, super bill, and/or charge sheet to identify the diagnoses and procedures and assigns ICD-10-CM codes to routine patient types.
  2. Identifies and assigns CPT-4 codes to all outpatient procedures.
  3. Abstracts diagnostic and procedural codes and other pertinent data into the network system as defined in policy and procedures.
  4. Reviews/monitors assigned work queues, physician notes reports, and missing documentation encounters and codes and abstracts any accounts that were missed.
  5. Provides information on specific problem accounts to the Coding Supervisor.
  6. Partners with the Coding Supervisor, Physician, and Practice Manager to identify and resolve documentation opportunities.

SYSTEM RESPONSIBILITIES

Safety:
Practices proper safety techniques in accordance with hospital and departmental policies and procedures. Responsible for the reporting of employee/patient/visitor injuries or accidents, or other safety issues to the supervisor and in the occurrence notification system.

Compliance:
Monitors and ensures compliance with all regulatory requirements, organizational standards, and policies and procedures related to area of responsibility. Identifies potential risk areas within area of responsibility and supports problem resolution process. Maintains records of compliance activities and reports compliance activities to the Compliance Office.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
